SWEEPSTAKES ENTRY PERIOD: The Jack Black Wax Pomade Survey Sweepstakes (the “Sweepstakes”) begins at 8:00 AM Central Time (“CT”) on July 21, 2015 and ends at 5:00PM CT on August 2, 2015 (the “Sweepstakes Period”). Sponsor’s computer is the official time keeping device for this Sweepstakes.

ELIGIBILITY: The Jack Black Wax Pomade Survey Sweepstakes (the “Sweepstakes”) is open to legal residents of the 50 continental United States, including the District of Columbia, age 18 or older at the time of entry. Void outside the U.S. and where prohibited by law. Employees of Jack Black, L.L.C. (the "Sponsor") and its parent, subsidiaries, affiliates, divisions, advertising and promotion agencies, and each of their respective officers, directors, shareholders, agents, employees, representatives, licensees, partners, associates, successors and assigns (collectively, "Sweepstakes Entities"), and the immediate family members of, and/or those living in the same household of each, are not eligible to enter. Sweepstakes is subject to all applicable U.S. federal, state and local laws and regulations. Participation constitutes each entrant’s full and unconditional agreement to be bound by these Official Rules and Sponsor's decisions, which are final and binding in all matters related to the Sweepstakes.

ENTRY: There are two (2) ways to enter. (1) After receiving an email invitation, follow the links and instructions to complete and submit the Jack Black Wax Pomade Survey in its entirety. Upon submission of the form, you automatically will receive one (1) entry into the Sweepstakes. Survey entries must be received between 8:00 AM CT on July 21, 2015 and 5:00 PM CT on August 2, 2015. (2) Mail-in Alternate Method: To enter without completing an online survey, hand print your name, address, day and evening phone numbers, email address (if any), age and “Jack Black Wax Pomade Survey 2015” on a 3″ x 5″ piece of paper and mail it in an envelope with proper postage to “Jack Black Wax Pomade Survey 2015,” Jack Black, L.L.C., 551 Southwestern Blvd., Suite 100, Coppell, TX 75019. You will receive one (1) entry into the Sweepstakes. All mail-in entries must be handwritten, postmarked by July 29, 2015 and received by August 3, 2015 . All entries become the exclusive property of Sponsor, and none will be acknowledged or returned. Proof of sending or submission will not be deemed to be proof of receipt by Sponsor. Sponsor is not responsible for lost, late, incomplete, invalid, unintelligible, illegible, misdirected or postage-due entries, which will be disqualified. Failure to comply with these instructions, fill in all required fields, provide true and accurate information and abide by these Sweepstakes Official Rules will result in disqualification.

LIMIT: Each participant may submit one (1) entry during the Sweepstakes Period, regardless of method. Online entrants are subject to all notices posted online, including, but not limited to, the Sponsor’s Privacy Policy. Multiple entrants are not permitted to share the same email address. Any attempt by any entrant to obtain more than the stated number of entries by using multiple/different email addresses, identities, registrations and logins, or any other methods will void that entrant’s entries and that entrant may be disqualified. Use of bots or any automated system to participate is prohibited and will result in disqualification. In the event of a dispute as to any entry, entries will be deemed to have been submitted by the Authorized Account Holder of the email address submitted at the time of entry. "Authorized Account Holder" means the natural person who is assigned to an email address by an Internet access provider, online service provider, or other organization that is responsible for assigning email addresses for the domain associated with the submitted email address. The potential winner may be required to show proof of being the Authorized Account Holder.

PRIZE: One (1) $100 of winner’s choice of Jack Black products will be awarded in this Sweepstakes. The gift card will be subject to the terms and conditions as set forth by the issuer of the gift card. Gift cards are not refundable or transferable, and may not be substituted or exchanged for cash or credit at any time. Lost or stolen gift cards will not be replaced. Approximate Retail Value (“ARV”): $100. The winner will be solely responsible for all taxes and all other fees and expenses not specified herein associated with the receipt and use of the Grand Prize. All prize details are at the Sponsor's sole discretion, and Sponsor reserves the right to substitute a prize of equal or greater value. Odds of winning will depend upon the total number of eligible entries received. Unclaimed prizes will not be awarded.

RANDOM DRAWING/NOTIFICATION: Sponsor will randomly select one (1) potential Grand Prize winner from among all eligible entries received, on or around August 3, 2015. Receiving a prize is contingent upon compliance with these Official Rules. The potential Grand Prize winner will be notified via email on August 3, 2015 after 9:00 AM CT. Winner is requested to confirm his/her mailing addresses within 48 hours via return email. The potential winners (or parent/legal guardian, if any winner is not of the age of majority in his/her jurisdiction of residence) may be required to sign and return an Affidavit or Declaration of Eligibility, Liability & Publicity Release (“Affidavit/Declaration”), which must be received by Sponsor within ten (10) days of the date notice or attempted notice is sent, in order to claim his/her prize. If any winner is not of the age of majority in his/her jurisdiction of residence, his/her prize will be delivered to such winner’s parent/legal guardian and awarded in the name of his/her parent/legal guardian. If any potential winner cannot be contacted within five (5) calendar days of first notification attempt, if any prize or prize notification is returned as undeliverable, if any winner rejects his/her prize or in the event of noncompliance with these Sweepstakes Official Rules and requirements, such prize will be forfeited and an alternate winner may be selected from all remaining eligible entries. Prize will be sent to the address on the entry form or address provided via email within 4-6 weeks of winner being verified. Upon prize forfeiture, no compensation will be given. Limit one (1) prize per person or household.

PUBLICITY: Except where prohibited by law, entry and acceptance of prize constitute permission for Sponsor to use winner's name, prize won, home state, likeness, photographs, and statements for purposes of advertising, promotion and publicity (including online posting) in any and all media now or hereafter known throughout the world in perpetuity, without additional compensation, notification or permission.

RELEASE: By participating, entrants and winners agree to release, indemnify and hold harmless Sponsor, Survey Monkey and their respective parent, subsidiary and affiliated companies, successors, assigns, partners, suppliers, manufacturers or distributors of Sweepstakes materials and advertising/promotion agencies and each of their respective parent companies and each such company’s officers, directors, employees and agents (collectively, the “Released Parties”) from and against any claim or cause of action, for any loss, harm, damage, injury, cost or expense whatsoever, including, without limitation, personal injury, death, or damage to or loss of property, arising out of or in connection with preparation for, travel to or participation in the Sweepstakes or possession, acceptance, receipt, use or misuse of any prize or participation in any Sweepstakes-related activity and for any claims based on publicity rights, defamation or invasion of privacy and merchandise delivery.
GENERAL CONDITIONS: Sponsor reserves the right, in its sole discretion, to cancel, terminate, suspend, extend and/or modify the Sweepstakes, or any part of it, if any fraud, technical failures, virus, bug, non-authorized human intervention or any other factor beyond Sponsor’s reasonable control impairs the integrity or proper functioning of the Sweepstakes, as determined by Sponsor in its sole discretion. In such event, Sponsor reserves the right to award the prizes at random from among the eligible entries received prior to and/or after (if appropriate) the action taken by Sponsor. Sponsor reserves the right, in its sole discretion, to disqualify any individual it finds to be tampering with the entry process or the operation of the Sweepstakes or any Sponsor websites or to be acting in violation of these Official Rules or any other promotion or in an unsportsmanlike or disruptive manner. Sponsor may prohibit an entrant from participating in the Sweepstakes or winning a prize if, in its sole discretion, it determines that said entrant is attempting to undermine the legitimate operation of the Sweepstakes by cheating, hacking, deception, or other unfair playing practices (including, but not limited to, the use of automated quick entry programs) or intending to annoy, abuse, threaten or harass any other entrants or Sponsor representatives. Any attempt by any person to deliberately damage Sponsor’s Page or any website or undermine the legitimate operation of the Sweepstakes may be a violation of criminal and civil law, and, should such an attempt be made, Sponsor reserves the right to seek damages from any such person to the fullest extent permitted by law. Sponsor’s failure to enforce any term of these Official Rules shall not constitute a waiver of that provision or any other rights that Sponsor may have.

LIMITATIONS OF LIABILITY: The Released Parties are not responsible for: (a) any incorrect or inaccurate information, whether caused by entrants, printing errors or by any of the equipment or programming associated with or utilized in the Sweepstakes; (b) technical failures of any kind, including, without limitation, malfunctions, interruptions, or disconnections in cellular networks, phone lines or network hardware or software; (c) unauthorized human intervention in any part of the entry process or the Sweepstakes; (d) technical or human error which may occur in the administration of the Sweepstakes or the processing of entries; or (e) any injury or damage to persons or property which may be caused, directly or indirectly, in whole or in part, from entrant’s participation in the Sweepstakes or receipt or use or misuse of any prize. If for any reason an entrant's entry is confirmed to have been erroneously deleted, lost or otherwise destroyed or corrupted, entrant’s sole remedy is another entry in the Sweepstakes. No more than the stated number of prizes will be awarded. In the event that production, technical, seeding, programming or any other reason causes more than the stated number of prizes as set forth in these Official Rules to be available and/or claimed, Sponsor reserves the right to award only the stated number of prizes by a random drawing among all legitimate, unawarded, eligible prize claims.

DISPUTES: Except where prohibited, entrant agrees that: (a) any and all disputes, claims and causes of action arising out of or connected with this Sweepstakes or any prize awarded shall be resolved individually, without resort to any form of class action, and exclusively by the state or federal courts located in Dallas County or the City of Dallas, Texas; (b) any and all claims, judgments and awards shall be limited to actual out-of-pocket costs incurred, including costs associated with entering this Sweepstakes, but in no event attorneys’ fees; (c) entrant waives all rights to trial in any action or proceeding instituted in connection with these Official Rules, including, without limitation, the Sweepstakes; and (d) under no circumstances will entrant be permitted to obtain awards for, and entrant hereby waives all rights to claim, indirect, punitive, incidental and consequential damages and any other damages, other than for actual out-of-pocket expenses, and any and all rights to have damages multiplied or otherwise increased. All issues and questions concerning the construction, validity, interpretation and enforceability of these Official Rules, or the rights and obligations of the entrant and Sponsor in connection with the Sweepstakes, shall be governed by, and construed in accordance with, the laws of the State of Texas, without giving effect to any choice of law or conflict of law rules (whether of the State of Texas or any other jurisdiction), which would cause the application of the laws of any jurisdiction other than the State of Texas.

PRIVACY: The information you provide will only be used to administer and execute this Sweepstakes and as otherwise permitted by Sponsor’s Privacy Policy, available at https://www.getjackblack.com/privacypolicy.aspx. By participating in the Sweepstakes, entrants hereby agree to Sponsor’s collection and usage of their personal information and acknowledge that they have read and accepted Sponsor’s Privacy Policy.
